Family-driven coordination for youth with complex needs

High-Fidelity Wraparound (HFW) is a coordinated, family-driven approach for youth with serious emotional or behavioral health needs, co-occurring substance use, or intellectual/developmental disabilities. The model brings families, providers, schools, child welfare, juvenile justice, and other supports together around one shared plan.

What This Service Does

• Gives families a central voice in identifying goals and choosing supports
• Coordinates services across behavioral health, education, child welfare, juvenile justice, and community systems
• Builds a team of natural and professional supports around the youth and family
• Creates one individualized plan focused on stability, strengths, and long-term success

Who It Serves

Youth and families facing complex needs that involve multiple providers or service systems and who would benefit from coordinated, team-based support.
